The client's card is a product that will make you see your client as a very professional person.

The card consists of an extensive questionnaire, which is a guarantee of safety for you. Signing a consent may protect us from unpleasantness in a situation when the client conceals from you an allergy to eyelash glue or other ailment that may be a contraindication to the treatment. The questionnaire also contains the content and permissions in accordance with the FAMILY.

Thanks to the tear-off part of the card, the client can take a leaflet with all the recommendations after the surgery.

The customer card consists of two parts:

An extensive questionnaire containing all the necessary information to perform the procedure, as well as the consent in accordance with the FAMILY.
Easily detachable part with recommendations after the procedure for the client.
