The card consists of an extensive questionnaire, which is a guarantee of safety for you. Signing a consent may protect us from unpleasantness in a situation when the client conceals from you an allergy or other ailment which may be a contraindication to the treatment. The survey also contains content and consents in accordance with the FAMILY.

Thanks to the peel-off part of the card, the client can take a leaflet with all recommendations after the surgery.
